Abstract Introduction: Pseudomonas aeruginosa display several resistance mechanisms to carbapenems and such variety makes it difficult to infer from the antibiogram. The aim of this study was to determine the carbapenem resistance genes in P. aeruginosa isolates with different profiles of phe-notypic susceptibility to these antibiotics. Materials and methods: From a microbial collection of P aeruginosa isolates from infected patients, 40 isolates with different carbapenem resistance profiles were selected. The carbapenemases genes, and expression of the OprD porin, the MexAB-OprM efflux pump and the p-lactamase AmpC were determined. Results: From a total of 40 isolates evaluted, in 21 (52.5%) any mechanism of resistance evaluated were detected. In the meropenem-resistant group, overexpression of AmpC (n = 1) and decreased expression of MexAB-OprM (n = 2) and OprD (n = 1) were found. A decrease in the expression of MexAB-OprM was observed in imipenem-resistant group (n = 3) and mutations in the gene encoding the OprD porin (n = 1). Finally, the presence of carbapenemases (VIM, n= 3, KPC-2 / VIM, n = 1) was detected in imipenem-meropenem resistant isolates. Conclusion: The phenotypic susceptibility profiles in P aeruginosa isolates were not explained by the molecular mechanisms explored, with the exception of carbapenemase-producing isolates. These results evidence the complexity of the antibiotic resistance mechanisms involved in this bacterium.


Resumen Introducción: Pseudomonas aeruginosa presenta diferentes mecanismos de resistencia a los carbapenémicos, dificultando su inferencia a partir del antibiograma. El objetivo fue determinar los genes de resistencia a car-bapenémicos en aislados de Pseudomonas aeruginosa con diferentes perfiles de susceptibilidad a estos antibióticos. Materiales y métodos: A partir de una colección microbiana de aislados de P. aeruginosa provenientes de pacientes infectados se seleccionaron 40 aislados con diferentes perfiles de resistencia a carbapenémicos y en los cuales se determinaron los genes de car-bapenemasas, la expresión de la porina OprD, la bomba de expulsión MexAB-OprM y la betalactamasa AmpC. Resultados: El 52,5 % de los aislados no presentó ninguno de los mecanismos de resistencia evaluados. En los resistentes a meropenem se encontró sobreexpresión de AmpC (n=1) y disminución de la expresión de MexAB-OprM (n=2) y OprD (n=1). En los resistentes a imipenem se observó disminución en la expresión de MexAB-OprM (n=3) y mutaciones en el gen que codifica la porina OprD (n=1). En aislados resistentes a imipenem y meropenem se detectó la presencia de carbapenemasas (VIM, n=3, KPC/VIM, n=1). Conclusión: Los mecanismos moleculares hallados no explican el fenotipo de resistencia a carbapenémicos, excepto en los aislados productores de carbapenemasas. Estos resultados evidencian la complejidad de los mecanismos implicados en la resistencia antibiótica en esta bacteria.

ABSTRACT

A leucemia mielóide aguda (LMA) é uma doença heterogênea em relação às características citogenéticas, moleculares, resposta ao tratamento, sobrevida global e livre de doença. A maioria dos pacientes apresenta recidiva precoce e evolui para o óbito devido à resistência à quimioterapia. O tratamento atual, baseado na associação das antraciclinas Idarrubicina (IDA) ou Daunorrubicina (DNR) com a Citarabina (Ara-c), se mantém por mais de 40 anos apesar dos resultados insatisfatórios. Objetivos: Avaliar os mecanismos de resistência aos quimioterápicos IDA, DNR e Ara-c, em linhagens celulares e em amostras de pacientes com LMA. Metodologia e Resultados: O perfil de resistência das linhagens HL60, Kasumi e U937 foi comparado por imunofenotipagem, citogenética, expressão e atividade das proteínas transportadoras de efluxo: glicoproteína P (Pgp), MRP1 e BCRP e expressão de proteínas envolvidas com a evasão da apoptose. Para avaliar a sensibilidade aos quimioterápicos foram utilizados o ensaio de viabilidade celular (MTT), fragmentação do DNA e ativação de caspase 3. As linhagens celulares foram comparadas, entre si em relação à sensibilidade e resistência aos fármacos. A IDA foi mais eficiente em reduzir a viabilidade celular na linhagem HL60, a IDA e a DNR na linhagem Kasumi, sendo o Ara-c o mais eficiente em reduzir a viabilidade na linhagem U937. O perfil de resistência da linhagem HL60R foi analisado através de um array de proteínas fosforiladas, onde encontramos diversas vias de sinalização inibidas na linhagem resistente como a via de sinalização das MAPKs JNK e p38, importantes para o processo de indução de apoptose pelos quimioterápicos. Foi verificado que a ativação de tais vias de sinalização pelos quimioterápicos esta diminuída nas linhagens de LMA resistentes em comparação com as linhagens sensíveis. As vias de sinalização de JNK e p38 nas linhagens de LMA foram inibidas com RNA de interferência. Tal inibição resultou na redução da sensibilidade aos quimioterápicos na linhagem Kasumi e U937, aumento da expressão da Pgp e do miR-27a. A análise das amostras de pacientes com LMA mostrou alta expressão do miR-27a quando comparadas com as amostras dos doadores saudáveis. No entanto, a expressão do miR-27a apresentou baixa correlação com a Ppg e diferenças na sobrevida global apenas em 12 meses após o tratamento. A técnica do CRISPR foi utilizada para editar/silenciar os genes JNK e p38 a fim de corroborar os resultados com o silenciamento. Nossos dados sugerem que alterações na fosforilação das MAPKs JNK e p38, envolvidas no processo de indução de apoptose, estão associadas à resistência aos quimioterápicos na LMA.

ABSTRACT

El hongo Macrophomina phaseolina (Tassi) Goid., agente causal de la enfermedad denominada «pudrición carbonosa¼, provoca pérdidas significativas en la producción de cultivos como maíz, sorgo, soya y frijol en México. Este hongo, parásito facultativo, muestra amplia capacidad de adaptación a ambientes estresantes, donde existen altas temperaturas y deficiencia hídrica, condiciones frecuentes en gran parte de la agricultura de dicho país. En este trabajo se describen algunos aspectos básicos de la etiología y la epidemiología de M. phaseolina. Asimismo, se revisa la importancia que guardan las respuestas de este hongo a ambientes estresantes, particularmente la deficiencia hídrica, de acuerdo con caracteres morfológicos y del crecimiento, así como fisiológicos, bioquímicos y de patogenicidad. Finalmente, se presentan algunas perspectivas de estudio del género, que enfatizan la necesidad de mejorar su conocimiento, con base en la aplicación de herramientas tradicionales y de biotecnología, y de dilucidar mecanismos de tolerancia al estrés ambiental, extrapolables a otros microorganismos útiles al hombre.


Fungus Macrophomina phaseolina (Tassi) Goid. is the causative agent of charcoal rot disease which causes significant yield losses in major crops such as maize, sorghum, soybean and common beans in Mexico. This fungus is a facultative parasite which shows broad ability to adapt itself to stressed environments where water deficits and/or high temperature stresses commonly occur. These environmental conditions are common for most cultivable lands throughout Mexico. Here we describe some basic facts related to the etiology and epidemiology of the fungus as well as to the importance of responses to stressed environments, particularly to water deficits, based on morphology and growth traits, as well as on physiology, biochemistry and pathogenicity of fungus M. phaseolina. To conclude, we show some perspectives related to future research into the genus, which emphasize the increasing need to improve the knowledge based on the application of both traditional and biotechnological tools in order to elucidate the mechanisms of resistance to environmental stress which can be extrapolated to other useful organisms to man.

Resumen Candida albicans es una levadura comensal capaz de causar una infección oportunista en hospederos susceptibles denominada candidiasis. El tratamiento para combatir la candidiasis puede ser tópico o sistémico según el tipo de infección, los antifúngicos más utilizados son los derivados imidazólicos (fluconazol, itraconazol, ketoconazol, miconazol etc.), sin embargo en la actualidad se observa una disminución en la efectividad de estos medicamentos, es decir, un fenómeno de resistencia de parte del microorganismo a estos fármacos, esto debido principalmente, al surgimiento de levaduras resistentes, a la aparición de nuevas especies patógenas, a la prescripción irracional de antimicóticos como profilaxis y al aumento de las dosis terapéuticas. Existen dos mecanismos por los que Candida puede adquirir resistencia a un azol. El primero es por mutaciones moleculares de la enzima diana del antifúngico, como la alteración de las enzimas relacionadas en la síntesis del ergosterol y el segundo por la alteración en las bombas de expulsión: ATP-binding cassette (ABC) y facilitadores mayores (MF). En este trabajo se resumen los principales mecanismos de resistencia en Candida y la importancia de hacer pruebas de susceptibilidad con el fin de brindar un tratamiento adecuado para este tipo de infecciones oportunistas.


Abstract Candida albicans is a commensal yeast capable of causing an opportunistic infection called candidiasis in susceptible hosts. Treatment to combat Candida may be topical or systemic according to the type of infection and the imidazole derivatives (fluconazole, itraconazole, ketoconazole, miconazole, etc.) are the antifungals most widely used. However, resistance to these drugs is observed by a decrement in their effectiveness. This is mainly due to the emergence of resistant yeasts and of new pathogenic species, as well as to the irrational prescribing of antifungal prophylaxis and the use of higher therapeutic doses. There are two mechanisms by which Candida can acquire an azole resistance, the first is by molecular mutations of antifungal target enzyme, as the alteration of enzymes related to the synthesis of ergosterols and the second by change in the efflux pumps, such as those of ATP-binding cassette and the higher facilitators. In this work the main mechanisms of resistance to Candida and the importance of performing susceptibility tests in order to provide an adequate treatment for this type of opportunistic infections are summarized.

Introducción: el programa de control de Aedes aegypti (Linnaeus) (Diptera: Culicidae) en Cuba utiliza temefos como larvicida y piretroides como adulticidas, aunque el organofosforado clorpirifos ha sido utilizado esporádicamente. Conocer el nivel de resistencia a estos insecticidas es esencial para lograr un control efectivo de esta especie. Objetivo: determinar el nivel de resistencia a insecticidas en su grado técnico y en sus formulaciones comerciales en Ae. aegypti de Pinar del Río. Métodos: una cepa de Ae. aegypti del Área de Salud Raúl Sánchez, Pinar del Río, fue evaluada a través de los bioensayos de la Organización Mundial de la Salud para determinar la susceptibilidad en larvas al organofosforado temefos en su formulación técnica. Se evaluaron además tres formulaciones granuladas de temefos (Abatex-G1, Biolarv G-1 y Temefar G-1). En el estado adulto se determinó el nivel de susceptibilidad a los insecticidas piretroides: cipermetrina, deltametrina, lambdacialotrina y al organofosforado clorpirifos, en su formulación técnica. Además se evaluaron algunos en su formulación comercial: Galgotrin 25 EC (cipermetrina), Aqua K-Otrina 2 EW (deltametrina) y Clorcide 44 EC (clorpirifos). Resultados: en larvas, se encontró alta resistencia a temefos, en su formulación técnica, y con los productos en su formulación comercial, se observó una efectividad del 100 por ciento, con recambio diario de agua, de hasta 20 días para Temefar G1, 18 días para Biolarv G1 y 12 días para Abatex G1. En los ensayos de adultos, la cepa resultó susceptible a cipermetrina, deltametrina y clorpirifos, y resistente a lambdacialotrina. Con respecto a las tres formulaciones comerciales evaluadas, solo se observó resistencia a Aqua K-Otrina 2 EW. Conclusiones: el uso de estrategias de control integrado de Ae. aegypti se hace necesario para disminuir la frecuencia de uso de temefos, y así recuperar la efectividad de este insecticida. Además, se evitaría la aparición de resistencia a productos adulticidas que aun mantienen su efectividad para el control efectivo de esta especie en la zona de estudio(AU)


Introduction: the control program of Aedes aegypti (Linnaeus) (Diptera: Culicidae) in Cuba uses temephos as larvicide and pyrethroids as adulticide although the organophosphorate chlorpyrifos has been barely used. The level of knowledge about resistance to insecticides is essential to effectively control this species. Objective: to determine the level of resistance to insecticides of Ae. aegypti from Pinar del Rio in its technical aspect and in commercial formulations. Methods: one Ae. aegypti strain from the health area Raul Sánchez in Pinar del Rio province was evaluated through the World Health Organization bioassays to determine susceptibility of larvae to temephos in its technical formulation. Additionally, three granulated formulations of temephos were evaluated (Abatex-G1, Biolarv G-1 and Temefar G-1). In the adult state, the level of susceptibility to pyrethroids called cypermethrin, deltamethrin, lambda cyhalothrine and to organophosphate chlorpyrifos in its technical formulation. Some of them were evaluated in its commercial formulation (Galgotrin 25 EC (cypermethrin), Aqua K-Otrina 2 EW (deltamethrin) and Clorcide 44 EC (chlorpyrifos). Results: it was found in larvae that the resistance to temephos was high in the technical formulation, but the commercial formulation showed an effectiveness rate of 100 percent., with daily change of water, up to 20 days for Temefar G1, 18 days for Biolarv G1 and 12 for Abatex G1. In the assays with adult vectors, the strain turned to be susceptible to cypermethrin, deltamethrin and chlorpyrifos and resistant to lambda cyhalothrin. Regarding the three evaluated commercial formulations, resistance to Aqua K-Otrina 2 EW was proved. Conclusions: the use of integrated control strategies for Ae. aegypti makes it necessary to reduce the frequency of use of temephos and to recover the effectiveness of this insecticide. Moreover, it will avoid the occurrence of resistance to adulticide products that are still effective for the control of this species in the study area(AU)

Introducción: el programa de control de Aedes aegypti (L.) (Diptera: Culicidae) en Cuba utiliza fundamentalmente temefos (ABATE) como larvicida y piretroides como adulticidas, y se ha utilizado, esporádicamente, el organofosforado (OF) clorpirifos. El monitoreo de la resistencia a estos insecticidas es esencial para lograr un control efectivo de esta especie. Objetivo: determinar la resistencia a temefos en larvas y sus mecanismos, y evaluar el nivel de resistencia a los insecticidas más utilizados como adulticidasen cinco áreas de salud del municipio Boyeros, La Habana, colectadas indistintamente en los años 2010-2012. Métodos: se evaluó la resistencia a temefos y la eficacia del mismo, en su formulación comercial (ABATEX G1), en larvas, a través de los bioensayos recomendados por la Organización Mundial de la Salud. Los mecanismos de resistencia se realizaron a través de ensayos bioquímicos. La resistencia en el estado adulto se determinó a través del método de las botellas impregnadas. Resultados: la resistencia a temefos en larvas disminuyó del año 2010 al 2012. El producto comercial de temefos mostró 100 por ciento de mortalidad entre 5 y 12 días. Se demostró que las esterasas y monooxigenasas desempeñaron un papel importante en la resistencia a temefos en larvas. En el estado adulto, se observó resistencia a piretroides y a clorpirifos en algunas cepas. Conclusiones: estos resultados corroboran la necesidad de establecer estrategias de control integrado para preservar la vida útil de los insecticidas disponibles para el control de Ae. aegypti en el municipio Boyeros(AU)


Introduction: the program for control of Aedes aegypti (L.) (Diptera: Culicidae) in Cuba is mainly based on the use of temephos (Abate) as larvicide and pyrethroids as adulticides. Organophosphate (OP) insecticide Chlorpyrifos has also been used on occasion. Monitoring resistance to these insecticides is essential to achieve effective control of the species. Objective: determine temephos resistance in larvae and its mechanisms, and evaluate the level of resistance to the insecticides most commonly used as adulticides in five health areas from the municipality of Boyeros, Havana, surveyed in the years 2010 and 2012. Methods: an evaluation was conducted of resistance to and effectiveness of temephos in its commercial formulation (Abatex G1) in larvae, using tests recommended by the World Health Organization. Resistance mechanisms were assessed with biochemical assays. The impregnated bottle bioassay was used to determine resistance in the adult stage. Results: larval resistance to temephos decreased from 2010 to 2012. The commercial product temephos showed 100 percent mortality between 5 and 12 days. Esterases and monooxygenases were found to play an important role in larval resistance to temephos. Some strains showed resistance to pyrethroids and Chlorpyrifos in the adult stage. Conclusions: these results corroborate the need to set up integrated control strategies to preserve the useful life of the insecticides available to control Aedes aegypti in the municipality of Boyeros(AU)

Abstract This study was conducted in four strains of Aedes aegypti mosquitoes to evaluate the enzymatic activity profiles in the city of Mossoró, Rio Grande do Norte, and correlate them with biochemical mechanisms of resistance to insecticides. Mosquitos were used to quantify the following detoxification enzymes: Mixed-Function Oxidase (MFO), PNPA-esterase (PNPA-EST), and Acetylcholinesterase (AChE). The profiles were compared statistically with profiles from the Rockefeller strain, through the Kruskal-Wallis test and Dunn's multiple comparisons (p < 0.05). The 99 percentile of the values of enzyme activity from the reference strain was calculated for each enzyme, and the percentage of individuals above the 99 percentile was quantified. The enzyme activities were classified as “Unchanged” (< 15%), “Identified change” (> 15% and < 50%), and “Substantially changed” (> 50%). The statistical analysis revealed significant differences in the MFO and AChE profiles, which are fundamental in the determination of profiles of resistance to insecticides. Three populations were classified as “Substantially changed” for MFO. The altered enzymatic activity showed that the changes could have an important role in exposing resistance to insecticides.


Resumo Este estudo foi realizado em quatro cepas de mosquitos Aedes aegypti da cidade de Mossoró, Rio Grande do Norte, com o intuito de avaliar os perfis de atividade enzimática e correlacioná-los com os mecanismos bioquímicos de resistência a inseticidas. Mosquitos foram utilizados para quantificar as seguintes enzimas de detoxificação: oxidase de Função Mista (MFO), PNPA-esterase (PNPA-EST) e acetilcolinesterase (AChE). Os perfis das populações foram comparados estatisticamente com os da cepa Rockefeller por meio do teste de Kruskal-Wallis e do de comparações múltiplas de Dunn (p < 0,05). O percentil 99 dos valores de atividade enzimática da cepa referência foi calculado para cada enzima, e o percentual de indivíduos acima desse valor foi quantificado. As atividades enzimáticas foram classificadas como “Inalterada” (< 15%), “Alteração Identificada” (> 15% e < 50%), e “Substancialmente Alterada” (> 50%). A análise estatística revelou diferenças significativas nos perfis de MFO e AChE, que são fundamentais na determinação de perfis de resistência a inseticidas. Três populações foram classificadas como “Substancialmente Alteradas” para MFO. Os níveis alterados de atividade enzimática demonstram que essa mudança pode desempenhar um importante papel na resistência a inseticidas.

ABSTRACT

Las Micobacterias de crecimiento rápido (MCR) son patógenos oportunistas capaces de ocasionar infecciones en piel, pulmonares y diseminadas. En Venezuela existe un incremento de estas infecciones derivadas de procedimientos invasivos, entre ellas cirugías estéticas. El tratamiento de esas infecciones requiere la administración de al menos dos antibióticos por varios meses. En la presente revisión se describe los mecanismos de resistencia a los antibióticos que se han reportado en las MCR, principalmente las tres especies que frecuentemente se han asociado a infecciones en piel: M. abscessus, M. chelonae y M. fortuitum. Los factores más importantes relacionados a la resistencia en los esquemas de tratamiento son: cambios relacionados en la permeabilidad de la membrana al antibiótico, la inactivación enzimática y modificaciones del sitio blanco. En MCR se han evidenciado la presencia de betalactamasas con actividad penicilinasas y cefalosporinasas y de acetil-transferasas que pueden modificar los aminoglicósidos. Las modificaciones de las regiones ARNr 23S y ARNr 16S han permitido explicar, en parte, la resistencia a macrólidos y aminoglicósidos. La metilación de los sitios blanco de acción de los macrólidos es otro factor importante, sobre todo en especies como M. abscessus. Éste último mecanismo explica la falla terapéutica que se ha reportado en pacientes tanto en infecciones pulmonares como en infecciones en piel. Es necesario ampliar el estudio sobre los mecanismos de resistencia en estas especies, tomando en consideración las complicaciones que llevan estas infecciones y los tiempos prolongados de tratamientos pudiendo generar efectos secundarios en los pacientes y la frustración cuando ocurre la falla terapéutica.


The rapid growing mycobacteria (RGM) are opportunist pathogens able to cause skin, pulmonary and disseminated infections. In Venezuela there is an increase in the prevalence of these infections especially following esthetic surgery. The treatment of such infections is cumbersome requiring the administration of at least two antibiotics for several months. In the present review we describe the principal antibiotic resistance mechanisms reported for RGM, particular for the species most frequently associated with skin and soft tissue infection: M. abscessus, M. chelonae and M. fortuitum. The most important factors associated with resistance to antibiotics are impermeability of the cell membrane, enzymatic inactivation and changes in the target site. In RGM the presence of the beta-lactamases with penicillinases and cephalosporinase activity has been demonstrated and the presence of acetyl-transferases which can modify aminoglycosides. Changes in the 23S rRNA and 16S rRNA regions has allowed to explain, partly macrolide and aminoglycoside resistance. Mutation in the macrolide target site is another important resistance mechanism, particularly for the species group M. abscessus. This last mechanism can explain treatment failure reported in the patients with pulmonary or skin infection. We conclude that there is a need for more extensive studies about the mechanisms of antibiotic resistance of RGM; taking into account the complications of these infections, the prolonged treatment long time, producing secondary effects and the frustrations of patient and physician caused by therapeutic failures.

ABSTRACT

Rice cultivars are affected directly and indirectly by the insect sogata. The mechanical damage or direct loss, is produced after feeding and ovoposition on the young leaves tissues, while the indirect damage is produced after the transmission of the Rice hoja blanca virus. We studied the morpho-anatomic structures associated with the resistance of the mechanical damage produced by the insect, in six rice cultivars, including controls for resistance and susceptibility (Makalioka and Bluebonnet 50), during August 2011, in Fundacion Danac, Venezuela. Samples were taken from leaf 3, where cuticle thickness, presence of macrohair, microhair and silica bodies in the second third of the leaf was evaluated. A significant difference to thickness of the cuticle, the presence of microhair in the leaves, and presence of silica bodies was observed among cultivars, determining a significant correlation among the number of macrohair and microhair in the adaxial leaf blade with the presence of silica bodies, and thickness of the cuticle and number of posed insects. Thickness of the cuticle and presence of silica bodies in the intercostals space of microhair and macrohair showed to be the variables most related to mechanical damage and resistance mechanism.


El arroz es afectado directa e indirectamente por el insecto sogata. El daño directo o mecánico es debido a la postura de huevos y alimentación del tejido foliar por parte del insecto y el daño indirecto que ocurre es por la trasmisión del virus de la hoja blanca del arroz. El ensayo fue conducido en la Fundación DANAC en agosto 2011, con la finalidad de evaluar las estructuras morfoanatómicas asociadas con la resistencia al daño mecánico en seis cultivares de arroz que incluyen a los testigos de resistencia y susceptibilidad (Bluebonnet 50 y Makalioka). En el tercio medio de la tercera hoja, se evaluaron las variables grosor de la cutícula, presencia de macropelos, micropelos y cuerpos silíceos. Se observaron diferencias significativas entre los cultivares para el grosor de cutícula, número de micropelos y presencia de cuerpos silíceos, se determinó una correlación significativa entre el número de macro-pelos y micropelos en la cara adaxial con la presencia de cuerpos silíceos y el grosor de la cutícula y el número de insectos posados a las 96h. El grosor de la cutícula y la presencia de cuerpos silíceos en los espacios intercostales de los micropelos y macropelos mostraron ser los caracteres más relacionados con la resistencia al daño mecánico y los mecanismos de resistencia al insecto.

Las infecciones asociadas a la asistencia sanitaria constituyen un grave problema de la salud pública a nivel mundial por su frecuencia y elevada mortalidad. El Acinetobacter spp. en la última década ha emergido como importante patógeno oportunista nosocomial. Dentro de estas especies, A. baumanii es la principal especie que se aisla hasta en 92 por ciento de las bacteriemias nosocomiales. La mayoría de los reportes de bacteriemia nosocomial por A. baumanii (B Ab) son de brotes en unidades de cuidados intensivos de pacientes adultos. Se ha reportado el incremento de la resistencia a antimicrobianos de este germen. Se realizó un estudio observacional descriptivo transversal acerca de la infección por Acinetobacter spp. en el Hospital Universitario Clínico Quirúrgico Comandante Faustino Pérez Hernández, del municipio de Matanzas, entre los meses de octubre de 2011 a julio de 2012. Teniendo en cuenta la ausencia de un estudio anterior en Matanzas sobre infección por este microorganismo, se decidió identificar la incidencia de Acinetobacter spp. según muestra biológica y servicio de procedencia e identificar la sensibilidad/resistencia del mismo, lo cual permitirá instaurar un tratamiento eficaz a los pacientes portadores de esta bacteria atendiendo al patrón de susceptibilidad encontrado en el estudio. La especie más frecuente fue A. baumanii, fundamentalmente en secreción endotraqueal y hemocultivo, procedentes en su mayoría de UTI, siendo este servicio el que aportó más cantidad de cepas MDR. Se encontró una mayor sensibilidad a antimicrobianos no considerados de primera línea como doxiciclina, tetraciclina y trimetropim-sulfametoxazol.


Infections associated to the sanitary care are serious public health problems at the international level because of their frequency and high mortality. In the last ten years, Acinetobacter spp. has emerged as an important nosocomial opportunistic pathogen. Among these species, A. baumanii (B Ab) is the main isolated species covering as many as 92 per cent of the nosocomial bacteremias. Most of the reports of nosocomial bacteremias by A. baumanii (B Ab) are outbreaks in adult patient intensive care units. It has been reported a boost of this germ antimicrobial resistance. We carried out a cross sectional, descriptive, observational study on Acinetobacter spp. infection, in the University Hospital Comandante Faustino Pérez Hernández of Matanzas municipality, from October 2011 to July 2012. Taking into account the absence of a previous study in Matanzas on infections caused by this microorganism, we decided identifying the Acinetobacter spp. incidence according to biological samples and coming-from service and identifying its sensibility/resistance, allowing the instauration of the efficacious treatment of the patients who carry this bacterium, considering the susceptibility pattern found in the study. The most frequently found species was A. baumanii, mainly in endotracheal secretions and hemo-cultures, most of them coming from Intensive Therapy Units, being this service the one contributing with more quantity of multidrug resistant stocks. We found a bigger sensibility to antimicrobials that are not considered first line ones like doxycycline, tetracycline and trimetoprim-sulfametoxazole.

ABSTRACT

Carboplatin is a derivative of cisplatin; it has a similar mechanism of action, but differs in terms of structure and toxicity. It was approved by the FDA in the 1980s and since then it has been widely used in the treatment of several tumor types. This agent is characterized by its ability to generate lesions in DNA through the formation of adducts with platinum, thereby inhibiting replication and transcription and leading to cell death. However, its use can lead to serious inconvenience arising from the development of resistance that some patients acquire during treatment, limiting the scope of its full potential. Currently, the biochemical mechanisms related to resistance are not precisely known. Therefore, knowledge of pathways associated with resistance caused by carboplatin exposure may provide valuable clues for more efficient rational drug design in platinum-based therapy and the development of new therapeutic strategies. In this narrative review, we discuss some of the known mechanisms of resistance to platinum-based drugs, especially carboplatin.


A carboplatina é um derivado da cisplatina, possuindo mecanismo de ação similar, diferindo em estrutura e toxicidade. Este fármaco foi aprovado pelo FDA em meados de 1980 e, desde então, tem sido amplamente usado no tratamento de diversos tipos de tumores. Este agente é caracterizado por sua habilidade em gerar lesões no DNA através da formação de adutos com a platina, inibindo a replicação e a transcrição, levando à morte celular. Entretanto, seu uso pode levar a graves inconvenientes, advindos do desenvolvimento de resistência que alguns pacientes adquirem durante o tratamento, limitando o alcance de seu potencial. Até então, os mecanismos bioquímicos relacionados ao problema da resistência não são precisamente conhecidos. Dessa forma, o conhecimento das vias associadas à resistência causada pela exposição à carboplatina pode prover valiosas informações para o planejamento racional de fármacos com base em platina mais eficiente e para o desenvolvimento de novas estratégias terapêuticas. Nesta revisão narrativa, serão discutidos alguns mecanismos de resistência a fármacos com base em platina, especialmente ao antitumoral carboplatina.

Acaricide resistance has become widespread in countries where cattle ticks, Rhipicephalus (Boophilus) microplus, are a problem. Resistance arises through genetic changes in a cattle tick population that causes modifications to the target site, increased metabolism or sequestration of the acaricide, or reduced ability of the acaricide to penetrate through the outer protective layers of the tick’s body. We review the molecular and biochemical mechanisms of acaricide resistance that have been shown to be functional in R. (B.) microplus. From a mechanistic point of view, resistance to pyrethroids has been characterized to a greater degree than any other acaricide class. Although a great deal of research has gone into discovery of the mechanisms that cause organophosphate resistance, very little is defined at the molecular level and organophosphate resistance seems to be maintained through a complex and multifactorial process. The resistance mechanisms for other acaricides are less well understood. The target sites of fipronil and the macrocyclic lactones are known and resistance mechanism studies are in the early stages. The target site of amitraz has not been definitively identified and this is hampering mechanistic studies on this acaricide.


A resistência aos acaricidas tornou-se amplamente difundida nos países onde os carrapatos bovinos, Rhipicephalus .Boophilus. microplus, são um problema. A resistência surge por meio de alterações genéticas em umapopulação de carrapatos que causam modificações no local de ação, aumento do metabolismo ou sequestro do acaricida, ou ainda redução na capacidade do acaricida em penetrar através das camadas protetoras do corpo do carrapato. Neste artigo, foram revisados os mecanismos moleculares e bioquímicos da resistência aos acaricidas que ocorrem em R. (B.) microplus. A partir de um ponto de vista dos mecanismos envolvidos, a resistência aos piretróides tem sido caracterizada em maior grau do que em qualquer outra classe de acaricida. Embora uma grande quantidade de pesquisas têm sido direcionada para a descoberta de mecanismos que causam resistência aos organofosforados, muito pouco é conhecido ao nível molecular, e essa resistência parece ser mantida por intermédio de um processo multifatorial e complexo. Os mecanismos de resistência para os demais acaricidas são bem menos compreendidos. Os alvos de ação do fipronil e das lactonas macrocíclicas são conhecidos, e os estudos dos mecanismos de ação envolvidos estão ainda em estágios iniciais. O alvo de ação do amitraz ainda não foi definitivamente identificado, e isso é limitante aos estudos dos mecanismos envolvidos na resistência a esse acaricida.

El Streptococcus pneumoniae, principal agente causal de la neumonía comunitaria, líder en la etiología de la otitis media y la meningitis, en las últimas 3 décadas ha incrementado, de manera importante, su resistencia a los agentes terapéuticos más utilizados, como los betalactámicos, macrólidos, azálidos y fluroquinolonas. La versatilidad adaptativa del microorganismo le ha permitido crear mecanismos capaces de sobreponerse a cualquiera de estas agresiones terapéuticas con un grado variable de eficacia. The Streptococcus pneumoniae, the main causal agent of community pneumonia, leader in the etiology of the otitis media and the meningitis, during the past three decades has increase in a significant way its resistance to the more used therapeutic agents including the beta-lactamase, macrolides, azalides and fluroquinolones. Las ß-lactamasas de espectro extendido (BLEE) son enzimas que fenotípicamente se caracterizan por conferir resistencia a penicilinas y cefalosporinas, incluyendo las de tercera y cuarta generación. Son en su mayoría producidas por enterobacterias especialmente frecuentes en Klebsiella pneumoniae y Escherichia coli. La identificación de este tipo de enzima es muy importante porque impone un plan de acción para el control de las infecciones que pudieran producirse. Por lo que se hace necesario diferenciar entre la producción de betalactamasas de espectro extendido (BLEE) y otros mecanismos de resistencia para evitar el tratamiento inadecuado de infecciones causadas por este tipo de cepas. Los procedimientos de laboratorio para la identificación de BLEE son muy importantes, existen varios métodos fenotípicos, de bioensayos, bioquímicos y genotípicos. No existe una metodología que aúne sensibilidad, especificidad y sencillez, dada la complejidad de la detección de las BLEE y sus variantes, por lo que se recomienda una adecuada interpretación de los perfiles de sensibilidad con los criterios habituales de lectura interpretada del antibiograma y posteriormente, se debe elegir el método de confirmación. Lo que permitirá conocer la verdadera dimensión del problema que representan, limitar su diseminación y adecuar las escasas opciones terapéuticas.


The ß-lactamasses of extended spectrum (ESBL) are enzymes that are phenotypically characterized to confer resistance to penicillin and cephalosporin, including those of third and quarter generation. They are mainly produced by Enterobacteriaceae especially frequent in Klebsiella pneumoniae and Escherichia coli. The identification of this enzyme type is very important because it imposes an action plan for the control of the infections that could take place. Therefore it becomes necessary to differ among the production of betalactamasses of extended spectrum (ESBL) and other resistant mechanisms to avoid the inadequate treatment of infections caused by this type of stumps. The laboratory procedures for the identification of ESBL are very important; there are several phenotype methods, biologic, biochemical and genotype assay. A methodology that joints sensibility, specificity and simplicity, doesn't exist given the complexity of the detection of the ESBL and its variants, thus an appropriate interpretation of the profiles of sensibility is recommended with the habitual approaches of interpreted reading of the antibiogram and later on, the confirmation method should be chosen . What will allow to know the true dimension of the problem that it represents, to limit their dissemination and to adapt the scarce therapeutic options.

Los glicopéptidos (vancomicina y teicoplanina) constituyen una alternativa terapéutica en el tratamiento de infecciones severas por cepas de S. aureus resistentes a meticilina. Sin embargo, ya se han descrito dos mecanismos de resistencia en S. aureus: resistencia de bajo nivel, caracterizada por un engrosamiento anormal de la pared celular, presente en las cepas VISA y, resistencia de alto nivel mediada por el operón vanA, que provoca la sustitución de los residuos terminales D-ala-D-ala por D-ala-D-lac, disminuyendo su afinidad por el antibiótico. Esta revisión resume la historia de la aparición de la resistencia a glicopéptidos en S. aureus y considera los mecanismos que determinan la resistencia en estos organismos como base para comprender la necesidad y los potenciales roles de nuevos agentes de esta clase


Glycopeptides (vancomycin and teicoplanin) are an alternative therapeutic in the treatment of severe infections by methicillin-resistant S. aureus strains. However, two resistance mechanisms of S. aureus have already been described: low-level resistance, characterized by an abnormal thickening of the cellular wall, present in the VISA strains, and high-level resistance, mediated by the vanA operon, which causes the replacement of D-ala - D-ala terminal residues by D-ala-D-lac, decreasing its affinity for the antibiotic. This review summarizes the history of the emergence of glycopeptide resistance in S. aureus and considers the mechanisms that determine the resistance in these organisms as a background for understanding the need and potential roles of new agents of this kind

S. aureus ha demostrado un gran poder de adaptación a los agentes antimicrobianos, adquiriendo paso a paso resistencia a todos los antibióticos disponibles para el tratamiento de las infecciones que ocasiona. Existen tres mecanismos de resistencia a los antibióticos ß-lactámicos en S. aureus: resistencia mediada por enzimas (penicilinasas o ß-lactamasas) las cuales desactivan al antibiótico; resistencia intrínseca, que no es debida a la inactivación de drogas y es responsable dela resistencia a meticilina; y la modificación de las proteínas de unión a penicilinas (PBPs). Además, S. aureus puede expresar el fenómeno de tolerancia, en el que ocurre una disociación de las acciones inhibitoria y bactericida de los antibióticos ß-lactámicos. De éstos, el mecanismo más importante, es la resistencia intrínseca que es probablemente más compleja, debido a que varios factores pueden afectar también su expresión


S. aureus has shown a great power of adaptation to antimicrobial agents, acquiring, step-bystep, resistance to all available antibiotics for treatment of the infections it causes. S. aureus has three major mechanisms of resistance to ß-lactam antibiotics: enzyme mediated (penicillinase or ß-lactamase) by which the antibiotic is inactivated; intrinsic resistance, which is not due to drug inactivation and accounts for methicillin resistance; and modifications of penicillin-binding proteins (PBPs). Additionally, S. aureus can express the tolerance phenomenon, in which there is a dissociation of the inhibitory and killing actions of ß-lactam antibiotics. Of these, the most important mechanism is intrinsic resistance, which is probably more complex because several factors can affect its expression
